Bridges to Health – WIP
We are a Through-The-Gate service working in HMP Bronzefield, HMP Downview and HMP Send, who work with women up to three months prior to leaving custody, supporting them to plan for their release and resettlement in to the community.
We then work Through-The-Gate providing them with support in the community for up to three months.
Support includes attending community appointments with them, referring them to support services including CMHT’s and adult social care, and linking them up with community services, including women’s centres, for continued support. Our community delivery area covers London and the South East, but we can work with women remotely who are being released beyond these areas.
Referral Information
We accept referrals from any professional in the prison, this includes mental health in-reach, healthcare, recovery, resettlement and the OMU.
All referrals must be made using the ‘Bridges to Health Referral Form’ and sent to the secure Bridges to Health email address. They will then be discussed as a team and allocated to an advocate.
We can accept referrals for women with a minimum of six weeks left in custody.
We can begin working with women up to three months prior to release and can accept referrals for women with more than three months left – these referrals will be placed on a wait list.
